Just for reference, open world map sizes:
GTA: San Andreas (36 km2)
Skyrim (39 km2)
Red Dead Redemption (41 km2)
Far Cry 4 (46 km2)
GTA 5 (81 km2)
Witcher 3 (136+ km2) *** this is from outdated info based on two regions, can anyone provide final map size? Researched but couldn't find

Evidently, FFXV will have an enormous map and seemingly much larger if this link is accurate (which it seemingly isn't, waiting for better info on this):

Witcher 3 isn't 136 km^2. The in-game Mini-Map provides meters so finding out world size is easy.
This is what I posted when B&W released:
I measured the in-game distances from N-S & W-E (the mini-map distances). The map is approximately 3085m X 2579m, or 7.96 km2. This is about 53% of the Novigrad/Velen/HOS total map size, and about the same size as all of the land area in Skellige (~35-40% of the full Skellige map, definitely a bit larger than Ard Skellig).
Note that this measurement (and the other map measurements) do not account for the restricted area at the border of each map where Geralt refuses to walk to. Accounting for those is a little difficult because the invisible border is not made up of straight lines. Mentally, I just tack off 10-15% of the sizes listed below.
Here are the rough calculations I did for the other maps:
Measuring the map from end to end (so including small restricted boundaries of each world map) gave me the dimensions, from which I got:
Novigrad & Velen (+HOS): 15.2 km2 (5.8 mi2)
Skellige Isles & Ocean: 28.9 km2 (11.1 mi2)
White Orchard: 0.8 km2 (0.3 mi2)
Kaer Morhen: 1.4 km2 (0.6 mi2)
Total: 46.3 km2 (17.8 mi2)
If we account for the boundaries it's probably a total right around 45 km2.
This was the best analysis of the Game Awards Map mathematically - everything is trig, only guess work is tree height & Link height as reference points. That yielded a size of ~240 sq. km for the Game Awards map (smaller than X which is ~400). Obviously it's tough to reconcile how close the Game Awards footage map is with what we saw today and what we'll have next Spring.
As for TP, my math yielded 1.04 sq km (.40 square miles).
Here's my map w/grid - each square is .01 sq km, so 10x10 squares is 1 sq km. Keep in mind that the 1.04 sq km is referring to green land/blue water only. Anything in black is not counted (so the area of the total rectangle is greater).
I really highly doubt that this new map is just 12x the size of TP. The portion we saw today itself looked significantly larger than TP. I want to see a direct quote from Aonuma. It may be possible he was referring to JUST the Plateau region as being ~12x TP size.
TP on Game Awards Map overlay (assuming 1.04 sq km for TP & 240 sq km for BotW):
Scale is 1 km. Each box is 1 sq km.
Also Witcher 3 overlaid on Game Awards Map (assuming 240 sq km). Doesn't include B&W.
Scale is 0.5 km. Each box is 0.25 sq km.

Nintendo has revealed that the open world in The Legend of Zelda: Breath of the Wild is 12 times larger than the map in Twilight Princess.
During a presentation at E3, Nintendo also revealed that the demo of Breath of the Wild shown at E3 -- which contains a large portion of open field as well as several enemies, Shrines, and more -- represents just 1% of the game’s total size.

After playing the game for about an hour, by our approximation, when fully zoomed out, each square on the map translates to roughly 1 square kilometre (0.4 square miles). The E3 demo is contained within a relatively small section of the map, a section that appears to be about 2 km by roughly 1.5 km. Though the size of the square grid on the map doesn’t scale with the map itself, the calculation is the same: the entire map appears to be roughly 360 square kilometres, or roughly 140 square miles.

This means that the new Zelda‘s map is just a tad smaller than that of Xenoblade Chronicles X, which has a map size of 154.44 square miles.

Xenoblade Chronicles X map is around 400 km2.
