Well, the previous three games were all rated T. The difference this time is that the guns feel much more punchy and as you can see in that GIF, the shotgun blast looks pretty brutal. Huh, I honestly don't remember shotguns throwing the thugs back that much in previous games. I guess it's a combination of the much improved gunplay and more realistic animations that makes it seem much more brutal in Uncharted 4 to me.Uncharted 1

Yeah, good point. Seems like blood flowing from enemies or being splattered on surfaces automatically nets an M rating on ESRB.I believe it's the handling of blood that can alter ESRB ratings. For example, Perfect Dark and Goldeneye both had blood. But only Perfect Dark had it where if you shot someone the blood would splatter onto a nearby wall. It ended up getting a M rating while GE received a T. As far as I know in Uncharted the blood doesn't stain the ground or walls. It comes out of enemies but just disappears afterwards.

Pretty much everything you just said, man. It's like watching Tom and Jerry whacking each other with hammers and then watching two real people doing it. Extreme example, but I'm sure it's the much more realistic animations that make it feel much more savage.Yeah, I think it's that.
They learnt a lot from The Last of Us in terms of animation and feedback. Animation is all about 'bounce and thud', and the ragdolls etc in Uncharted 4 look best in class for that. Hugely more so than UC 1-3. The animation (from these vidoes) for when he snaps a neck and drops the guy's body to the floor looks really brutal just because it has that real-life 'thud' to it. I think that's what's jarring you here.

In any case, I couldn't get used to the shotgun in the UC4 beta, had to use AR or Sniper.

Could be. I mean Quantum Break just released and it's rated M for...
From my playthrough I don't recall any blood spatter sticking to objects and there only were a few instances of "strong language". There are no decapitations either but the guns and hit animations are pretty intense though.

It's not 60 frames but incredible how close it got to the reveal in the end since it looks better than most 30 fps games currently out.

As far as the water is concerned, most vast water systems these days are tessellated. This includes The Witcher 3 and Uncharted 3, but goes back as far as Crysis 1. This is important so that you don't have super-dense mesh from far away - it's modern LOD and extends to things like character and object silhouettes. (Extra: post with details about U4 from Edge)
Soft-body physics are definitely a part of the game, both in pre-defined manner and runtime. They even showed this in a recent presentation (about 34 min in) up in San Francisco at GDC where they showed vehicle deformation using this implementation. The physical impact of the vehicle traction and how it affects the characters is an extension of this, and deformation of rigid and soft-body elements are all over the Sam-Pursuit video from E3 2015.
The whole day/night cycle stuff I can't vouch for, but considering the vastness of the game they almost definitely have a GI system in place as it cuts down a LOT of time needed to make and place assets and with PBR it means weather changes wouldn't require a complete redo of all assets. It felt like GI in the multiplayer as well. Different time of day in the same location is also easily achieved this way, though baked lighting and predefined lighting setups make sense in cutscenes.
