He mentions stuff about over the air TV, whcih would not be using HDMI outputs, and therefore can't be used, which isn't too uncommon in many places. Also many cable providers are still using component outputs for their boxes, once again of no use for the X1.
Canada, for example has Rogers and Bell for their most common TV providers (there are a few more, but tend to be more region specific) They have boxes that use HDMI, but many of the older boxes still only have component connections. You'd need a new box for it to work.
He's basically speaking about the cable box availability. Or at least that what I understand. Many European countries are using HDMI TV antennas and wouldn't be able to use the TV features.

I can only speak from my country, but here in Denmark people with over the air TV still has a box connected that does HDMI, and here it is either terrestial cable directly into the TV or a box either connected to over the air or IPTV.

To counter this kind of bad announcement, they could always explain how the IR blaster features and HDMI pass through are going to work. Is it going to be possible to get it to work with any HDMI set-top box? Will it have presets for popular ones? Will it be properly configurable? Given the silence, I assume it's going to be a bit of a half-arsed non-feature.
Aside from Sky, there are other expanding TV services in the UK that use HDMI set-top-boxes. For example, YouView, a collaboration between the UK's four terrestrial broadcasters and telecoms companies. Mind you, as YouView is non-profit, like BBC's iPlayer, Microsoft might not be able to get away with locking it behind XBL Gold.
As a huge fan of the old Xbox and the 360, I've STILL got an Xbox One pre-order in. But with the complete lack of info on the few games I was interested in (Forza, Quantum Break) and the constant PR screw ups, I'm getting closer and closer to cancelling it. If it were still possible to pre-order a PS4 for launch day in the UK, I'd probably switch today.

This would also be the case for the Xbox One Guide; there are countless mobile apps and TV guide websites that must be automatically ingesting programming data from somewhere. With that in mind it seems mad for Microsoft not to launch with at least the Guide in their biggest European territory. Also mad for Penello not to give a positive answer explaining the ways you CAN use TV pass-through in non-fully-supported countries. Like you say, if it is a non-starter outside of the US and Japan, it's very disappointing.All of that data is readily available to license centrally, as are IR databases for the set top boxes - my WiiU can turn my TV over so if Nintendo can do it there is no excuse for MS

I don't think they're neccesarily focusing on japan. Its just that the broadcast rules there are so laxed. Dvd tv recorders were never even available here while everyone in japan had one.Seems futile to put focus on Japan, but, gotta say, it's admirable.
